Financial Crimes Unit: Merchants Form

Definition:

This is a form which provides us with information we need to generate an offense.

You MUST provide us with a Merchants Form with each forged item you submit to us. For example, if you have 10 forged items, you must submit 10 merchant forms, one attached to each forged item.

The Merchants Form form requires the Adobe Acrobat Reader plug-in.

After downloading and printing the form, complete the form and return it to:
